FINANCIAL POLICY
PAYMENT IS DUE WHEN SERVICES ARE RENDERED UNLESS ARRANGEMENTS HAVE BEEN MADE IN ADVANCE.
We make every effort to provide affordable Chiropractic care to everyone who requests it. For this reason, we provide the following options:
OPTION #1 Cash, check or credit cards (Master Card, VISA, Discover) at the time services are rendered.
OPTION #2 Insurance non-assignment -- in this case we will provide you with a statement for services and you can file them directly with your insurance company. They will forward insurance benefits directly to you. Regardless of what your insurance does, you are responsible for the fee for the services using one of the above
methods of payment.
OPTION #3 Personal Injury Cases -- We will gladly bill your personal injury protection carrier, then your health insurance company. In the event you seek settlement through a third party or legal counsel we will require monthly payment from you until settlement of your case.
OPTION #4 Workers Compensation Cases -- We will gladly bill your worker's compensation injury protection carrier.
IT IS IMPORTANT TO KNOW THAT YOUR HEALTH INSURANCE CONTRACT IS BETWEEN
YOU AND YOUR INSURANCE COMPANY AND COVERS HEALTH CARE ONLY UNTIL SYMPTOMS ARE RELIEVED AND YOUR CONDITION HAS STABILIZED TO A PRIOR STATE.
INSURANCE COVERAGE CAN EXPIRE BEFORE YOUR HEALTH CARE IS COMPLETE, IF DEEMED RECONSTRUCTIVE, SUPPORTIVE, or WELLNESS CARE BY YOUR INSURANCE COMPANY.
ANY OF THE ABOVE OPTIONS MAY BE USED TO TAKE CARE OF OUR FEES FOR SERVICE. IF YOU HAVE ANY QUESTIONS REGARDING OUR OPTIONS, PLEASE FEEL FREE TO DISCUSS THIS WITH US.
CREDITS LESS THAN $3.00 WILL BE VOID AND REVERSED ON EACH INDIVIDUAL INACTIVE ACCOUNT GREATER THAN 6 MONTHS FOR ACCOUNTING PURPOSES.

Harmon Family Chiropractic believes in the value of chiropractic care for you and your family, maintains a pricing structure that allows care for all. Our office participates in most insurance programs. We also accept auto accident and workers compensation cases. For patients who have little or no chiropractic insurance coverage, flexible payment programs can be arranged. Insurance
Most insurance plans are accepted at our office. We accept auto accident, workers compensation, personal injury and most health insurance plans, including Medicare. Since there are so many plans and coverage varies widely, please call us to find out if your plan covers care in our office. Our staff will verify and explain your benefits before treatment, at no charge. However, we are not a Medicaid provider.
No Insurance
For patients who have little or no chiropractic insurance coverage, flexible payment programs can be arranged. If you have no insurance or do not have chiropractic benefits there is still a way for you to receive the care you need. Many patients pay directly for care, as they discover chiropractic to be extremely cost-effective and affordable.

We assist and guide you to take the time to care for the important people in your world: you and your family. In an effort to make living the wellness lifestyle as easy and affordable as possible, we create a customized treatment plan that is specific to you and your family's needs.
Health on a Budget
At Harmon Family Chiropractic we're convinced that a small amount of your time and money to care for your most important asset--your health--will be far less time consuming and expensive than trying to recover it. If you feel you do not have enough time or money to take care of yourself, please contact us--we can help! The last thing we want is for you to come to our chiropractic center in crisis. So please, we would like to encourage you to take your health very seriously. As the old saying goes, if you don't have your health, you don't have anything. Unfortunately, too many people don't discover how true that statement is until it is too late.
